Free sports physicals for students in grades 6 through 12 will be offered Tuesday, June 4, at Siuslaw Middle School, sponsored by PeaceHealth Medical Group.

"PeaceHealth Medical Group will contribute medical supplies and the time and expertise of five providers and several medical assistants for the sports physical," according to a press release by PeaceHealth.

School Sports Physical Day, for both Siuslaw and Mapleton students, offers physicals required for participation in both school and youth leagues.

Students will receive a preparticipation examination that includes height, weight, blood pressure, pulse check and eye examination.

Examination forms are available at each student's school. Forms must be completed and signed by a student's parent and/or guardian prior to the examination.
