Sports Physical Day for Siuslaw Vikings is Wednesday, June 7, at Siuslaw Middle School, high school athletic director Chris Johnson announced.

Incoming fifth, seventh, ninth and 11th graders should have a physical, plus all students out of this cycle, Johnson said.

Physicals are good for two years.

Students can get a physical examination form from the school office or online at http://www.osaa.org/docs/forms/PhysicalExamination-2017.pdf.

The new form from the Oregon School Activities Association must signed by both the parent and the student and brought to the examination.

The schedule for examinations:

• 8:15 a.m. — Fifth graders
• 9:15 — Sixth graders
• 9:30 — Seventh graders
• 9:45 — Eighth graders
• 10 — Freshmen
• 10:15 — Sophomores
• 10:45 — Juniors
• 11 — Catch up and make up

Physical examination for Mapleton students is 8:45 a.m. the same day at the Siuslaw Middle School.
